Form 1095-C’s are now available upon request. This form shows the health coverage offered to you and the months you were enrolled, if applicable, and is reported to the IRS.
Most people do not need Form 1095-C to file their federal tax return. However, you may need it in certain situations, such as applying for coverage through the Health Insurance Marketplace.
